Service Overview

1. Domiciliary Care (Dom Care)

A licensed, Medicaid waiver-funded program through the Philadelphia Corporation for Aging (PCA) providing room, board, and supervision in a family-like setting.


Services include:

  •  24-hour supervision and assistance
  • Personal care (bathing, grooming, toileting)
  • Medication reminders/management
  • Meal preparation and dietary monitoring
  • Social/recreational activities
  • Transportation coordination
  • Coordination with case managers, PCA, and CHC-MCOs


Target Population:

  • Adults 18+ who are elderly or disabled and require supervised living but not a nursing home


Funding:

  •  Community HealthChoices (CHC) Waiver
  • Optional private pay supplement

Shared Housing (Unlicensed Supportive Living)

An affordable housing model where multiple unrelated residents share a home with support available as needed.


Services include:

  •  Safe, stable housing
  • House management and supervision
  • Life skills coaching (e.g., budgeting, hygiene, chore
  • Peer companionship
  • Conflict resolution & mediation
  • Assistance with benefits and community resources
  • Service coordination with third-party providers


Ideal for:

  •  Individuals stepping down from inpatient or residential care
  •  Those in recovery, reentry, or needing transitional support

Supportive Housing (Waiver-based or Grant-funded)

Designed for individuals needing stable housing and wraparound services due to mental health, developmental, or physical disabilities.


Services include:

  •  Individualized service plans (ISPs)
  • Case management
  • Crisis intervention coordination
  • Coordination with clinical and non-clinical providers
  • ADL/IADL support based on assessed need
  • Community integration and social supports


May be funded by:

  • Medicaid Waivers (e.g., CHC, OBRA, AAW, ID/A)
  • State housing programs or reentry grants

Adult Foster Care / Life Sharing (when eligible)

Medicaid-funded, home-based care model where residents live in a provider’s home or a group-living setting under a person-centered model.


Key features:

  •  Caregiver lives on-site or is present regularly
  • Close, long-term living relationships
  • Highly individualized support in a home environment

Transitional Housing Services

 Short-term housing and support services for individuals exiting institutions, homelessness, or unstable living situations.
